Conducts nursing assessments, makes nursing diagnosis, and plans, implements, and evaluates the plan of care, including eventual discharge, for clients.

• Organizes home care services within the home in order to successfully manage the client's needs.
• Assesses the needs of persons who cannot function independently in the community and of their families.
• Delivers the appropriate therapeutic component of professional nursing service in accordance with professional practice standards, to clients and families.
• Mobilizes and coordinates other required health and social services to permit clients and their families to function as independently as possible.
• Authorizes medical supplies and equipment from noninsured services.
• Promotes the philosophy of selfcare and independence by providing instructions to individuals, families, and other groups of people
• Develops, reviews, reassess and revises, on an ongoing basis, care plans, with the client, the family and other care providers.
• Provides information and teaching to clients, or care givers, on self care, preventative, rehabilitative and comfort measures.
• Reviews case workload with home care coordinator on a regularly scheduled basis to determine priorities and make referrals as necessary.
• Maintains client records in approved formats, ensures that documentation meets legal, medical.
• Undertakes quality assurance measures to ensure that program operations are conducted within the professional standards, guidelines, and limits of Federal and Provincial regulations and to determine if client care plans are being carried out in accordance with standards of care and best practices.
• Assists in the preparation of appropriate policy and procedures pertaining to the provision of home care services and ensures that approved policy and procedural guidelines are maintained and followed.
• Promotes the philosophy, goals, objectives, and activities of the SCFN home care program.
• Monitors public policy/procedural issues which may have an impact on the program.
• Liaises with various departments and government agencies, attends meetings as required and sits on various committees.
