    Beschreibung

    Care Coordination Specialist

    We are seeking a dedicated professional to manage patient care files and provide quality nursing services in the comfort of patients' homes.

    • Nurse caregivers will work closely with healthcare teams, coordinating medical procedures and collaborating with stakeholders involved in patient care.

    The ideal candidate should possess strong communication skills, be organized, and have the ability to multitask effectively. Additionally, they should be able to maintain confidentiality while providing personalized attention to each patient's needs.


    • High school diploma or equivalent required; bachelor’s degree in Nursing (BSN) an asset;
      • ​
    • Current certification as a Registered Nurse (RN);
      • ​
    • A minimum 1 year experience working as an RN; ;
      • ​
    • Familiarity with long-term home care programs preferred but not essential;
