icare is currently seeking an experienced Case Manager with a passion for social impact to join the Lifetime Care team. Assist in ensuring people with severe injuries can participate in their communities Permanent opportunity Salary from $104,539 plus superannuation Were looking for someone with knowledge, practical experience, and skills, to support participants and their families, post rehabilitation as they transition back into their community following serious injury, including brain and spinal injuries. Playing an integral part in participants lives by understanding their needs, youll source, identify, and facilitate access of services and support requirements and continue to manage and monitor, focusing on ensuring they are maximising their independence and community engagement. Youll be working with a diverse mix of creative, curious and smart people who really care about making a difference for people and the NSW community. Your team will include supportive allied health professionals and as part of your continuing career development, you will be provided with ongoing professional development and peer support. You will have a mind for thinking holistically and a heart for people and you will set the bar high through the quality of the work you do and the people-oriented decisions you make. From an allied health background or rehabilitation advisor experience you will be managing a caseload of participants with complex injury related needs. You will need to determine treatment planning needs and coordinate the relevant services, support and monitor the implementation of funded services. Use your critical reasoning skills to interpret the relevant legislation to support decision making in relation to funding injury related services, ensure a high degree of integrity in the decision-making process and your excellent communication to build rapport and trust. A full clean drivers licence and the ability to travel regularly within NSW and interstate is required.
