Mental Health Care Coordinator Nurse

At our aspmad-cms-nord-vaudois, we're seeking a Mental Health Nurse with advanced skills in clinical, interprofessional, and leadership roles.

Your Mission:

• Conduct regular assessments to adapt care to evolving client needs.
• Work synergistically with multidisciplinary teams to optimize support.

In your role as resource person, you'll provide guidance and support to colleagues while acting as a leader within the team. You will also organize personalized care plans for clients and maintain up-to-date records according to protocols.
